Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking a Community Integration Counselor in Mechanicsburg, PA. Responsibilities
  • Assist individuals with learning physical, intellectual, emotional, and social life skills
  • Provide individualized services to manage and overcome community challenges
  • Collaborate with case managers to develop and facilitate programs meeting individual needs
  • Support community integration efforts and document progress
  • Build a support network between individuals and the community
  • Ensure medical consultation forms are completed and administer medications
  • Coordinate leisure and volunteer opportunities and monitor health and medical needs
  • Drive individuals to medical appointments and activities
  • Ensure program documentation and billable records are completed accurately and timely
Requirements
  • High School Diploma or GED
  • Bachelor's Degree preferred
  • Valid driver's license with a satisfactory driving record
  • Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
  • Ability to establish a comfortable and supportive relationship with individuals receiving supports
  • Ability to exercise good judgment and remain calm in crisis situations
  • Excellent verbal and written communication skills
  • Excellent time management skills
