Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states. We are seeking a Community Integration Counselor in Mechanicsburg, PA. Responsibilities- Assist individuals with learning physical, intellectual, emotional, and social life skills
- Provide individualized services to manage and overcome community challenges
- Collaborate with case managers to develop and facilitate programs meeting individual needs
- Support community integration efforts and document progress
- Build a support network between individuals and the community
- Ensure medical consultation forms are completed and administer medications
- Coordinate leisure and volunteer opportunities and monitor health and medical needs
- Drive individuals to medical appointments and activities
- Ensure program documentation and billable records are completed accurately and timely
- High School Diploma or GED
- Bachelor's Degree preferred
- Valid driver's license with a satisfactory driving record
- Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
- Ability to establish a comfortable and supportive relationship with individuals receiving supports
- Ability to exercise good judgment and remain calm in crisis situations
- Excellent verbal and written communication skills
