About the Role

Maximus is seeking a skilled Field Registered Nurse Assessor to provide high-quality home assessment services to vulnerable populations in the Bronx. As a key member of our team, you will conduct assessments to determine eligibility for community-based long-term services, utilizing the UAS-NY assessment tool.

Key Responsibilities
  • Travel to consumer homes, facilities, or conduct telehealth assessments as needed.
  • Perform in-home, in-facility, or telehealth assessments, evaluating individual health status, strengths, care needs, and preferences.
  • Establish and maintain positive relationships with consumers, caregivers, and facility staff.
  • Regularly review medical documentation, reporting concerns, conflicting information, or issues that may arise during the evaluation process.

Essential Duties and Responsibilities
  • Serve as the lead content knowledge expert concerning the review and research of provided clinical information.
  • Perform clinical case reviews and consultations.
  • Provide direct supervision to assigned team of nursing and/or other clinical staff.
  • Provide subject matter expertise related to medical necessity and level of care criteria and application thereof.
  • Enter assessment data into the UAS-NY online application and transmit, as required.
  • Code the UAS assessment correctly based on training and guidelines in the UAS-NY Community Health Assessment Reference Manual.
  • Review non-qualified assessments with the Clinical Quality Assurance Department.
  • Maintain a comprehensive working knowledge of community resources, payor requirements, and network services for target population.
  • Maintain updated knowledge of the UAS-NY assessment process.
Requirements
  • Current Registered Nurse (RN) license valid in the state of practice.
  • Bachelor's degree and 5 years of experience or equivalent combination of education and experience required.
  • Minimum of 5 years of clinical experience.
  • Minimum 1 year supervisory experience preferred.
