Job Summary

We are seeking a skilled occupational therapist to join our team in Blaine, MN. As an occupational therapist with Adara Home Health, you will have the opportunity to work with clients in their homes, providing personalized care and support to help them achieve their goals.

Responsibilities
  • Administer occupational therapy evaluations and participate in the development of the Plan of Care.
  • Establish reasonable treatment goals and keep treatment care plans current, reassessing clients and making recommendations as needed.
  • Provide occupational therapy treatments, procedures, evaluations, and diagnostic tests for clients under the direction of their physician.
  • Translate exercises into functional activities or activities of daily living.
  • Provide instructions in the use and care of special equipment when necessary.
  • Develop plans for modifying equipment, appliances, and the physical surroundings in clients' homes.
  • Prepare clinical and progress notes, reporting to physicians and the Area Clinical Manager on clients' responses to treatment or changes in condition.
  • Maintain appropriate records, including frequency of visits and clients' responses.
  • Instruct and supervise COTAs, HHAs, LPNs, and RNs in therapy programs when appropriate.
  • Provide in-service education programs for company employees as requested.
  • Participate in case conferences as indicated.
  • Provide families with information, support, and encouragement to help motivate clients in their progress, instructing clients' families on home therapy programs.
  • Assist individuals and families in accepting and adjusting positively to physical, mental, and social limitations.
  • Participate in the development of community resources to meet clients' needs.
  • Maintain positive and effective communication with all employees and others, understanding and participating in the team concept.
  • Maintain absolute confidentiality of all information pertaining to clients, families, and employees.
  • Maintain a safe client environment and identify and report to the office any suspected vulnerable client abuse, neglect, or financial exploitation.
  • Maintain productivity as per guidelines.
Qualifications
  • Currently licensed as an Occupational Therapist in the state of Minnesota.
  • Graduate of AOTA-approved school of occupational therapy and currently certified by the AOTA to practice occupational therapy.
  • Have at least two (2) years of experience in a healthcare setting or equivalent experience, with home care experience preferred.
  • Certification with the National Board of Occupational Therapy is strongly recommended.
  • Strong interpersonal communication and teaching skills.
  • Strong organizational skills and the ability to work independently.
  • Excellent written and oral communication skills.
  • Access to a dependable vehicle or public transportation to travel to multiple company business stops a day.
  • A valid driver's license and proof of car insurance when using a personal vehicle for company business.
  • Proof of negative mantoux or documentation of negative chest x-ray.
  • Current CPR certification is recommended.
  • Have U.S. Citizenship or evidence of valid Alien Work Permit.
  • Disclose any conviction and criminal history records pertaining to any crime related to the provision of health services.
