The Registered Nurse (RN) is a vital member of our interdisciplinary team, providing skilled professional nursing care to terminally ill patients in their place of residence. As a Hospice Case Manager RN, you will be responsible for delivering high-quality, patient-centered care that meets the unique needs of each individual.
Key Responsibilities- Provide skilled professional nursing care to assigned patients, as prescribed by the physician and in compliance with the standards of the Missouri Nurse Practice Act.
- Conduct thorough assessments of patients' physical, emotional, and spiritual needs, and develop individualized care plans to address these needs.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and effective communication.
- Administer medications, treatments, and other interventions as ordered by the physician, and document all care provided.
- Provide education and support to patients and families on end-of-life care, pain management, and other relevant topics.
- Participate in interdisciplinary team meetings to discuss patient care and develop strategies for improving patient outcomes.
- A nursing degree from an accredited school.
- A current RN license in the state of Missouri.
- BLS CPR Certification through American Heart Association required.
- Recent hospice or long-term care experience preferred.
