Optum is seeking a Part Time Nurse Practitioner - Traveler to join our HouseCalls team in Louisiana.

As a clinician-led care organization, we are creating a seamless health journey for patients across the care continuum. Our Home and Community Care team provides annual clinical assessments to patients in the comfort of their homes, identifying and reducing health risks, and coordinating follow-up care to improve health and well-being.

Our integrated medical teams practice within Home and Community Care, bringing high-end medical service, compassionate care, and industry-leading solutions to our most vulnerable patient populations. We address the physical, mental, and social needs of our patients, helping them access and navigate care anytime and anywhere.

We offer flexible scheduling, with no on-call, weekends, or holidays required. Travel expenses are covered, and you'll receive a base salary plus multiple incentives.

Key Responsibilities:
  • Conduct in-home assessments, including evaluations of past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exams.
  • Perform evidence-based practice screenings, including point-of-care testing (as appropriate).
  • Identify diagnoses to be used in care management and active medical management in the furtherance of treatment.
  • Formulate a list of current and past medical conditions using clinical knowledge and judgment and the findings of your assessment.
  • Communicate findings in your assessment to inform the member's PCP of potential gaps in care.
  • Identify urgent and emergent situations and intervene appropriately.
