Job Summary
We are seeking experienced Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our team as Pediatric Home Care Nurses. As a Pediatric Home Care Nurse, you will work one-on-one with our clients, providing personalized care and support to help them achieve their full potential.
Responsibilities
- Provide skilled pediatric care to clients in their homes, including medication management, wound care, and vital sign monitoring
- Develop and implement individualized care plans to meet the unique needs of each client
- Collaborate with healthcare teams to ensure seamless care transitions and coordination
- Conduct thorough assessments and evaluations to identify client needs and develop effective care strategies
- Document client progress and communicate with healthcare teams to ensure continuity of care
Requirements
- RN/LPN with a current valid license to practice as issued by the State Board of Nursing
- Experience working with pediatric clients, preferably in a home care setting
- Strong communication and interpersonal skills, with the ability to work effectively with clients, families, and healthcare teams
- Ability to work independently and as part of a team, with a focus on delivering high-quality patient care
