We are seeking a skilled LPN Assistant Clinical Coordinator to support our healthcare team at Haven Hospice. As a key member of our clinical staff, you will play a vital role in scheduling staff, verifying physician orders, and ensuring the accuracy of patient information.


Key Responsibilities



  • Schedule CNA visits for the week in advance, utilizing electronic health records
  • Verify physician orders and send complete orders for MD signature
  • Participate in morning Access calls in the absence of the Clinical Supervisor
  • Assist with physician verbal orders, document as necessary, and triage RN/SW calls when required
  • Review and track lab results, fax to MD, and place copies in the RN box
  • Communicate patient information and changes to Nurse Triage via email
  • Administer PPD or other vaccinations for staff as needed
  • Manage orders for equipment and IV meds/supplies from contracted vendors
  • Assist with clinical issues when the Clinical Supervisor is out of the office
  • Provide necessary information to physicians, patients, family members, team members, and other callers
  • Inform the Clinical Supervisor of unusual or potentially problematic patient/family issues

Requirements



  • High School Diploma or GED equivalent required
  • Advanced education for clinical licensure, with successful completion of IV therapy course (30 hours)
  • Minimum of 2-3 years nursing experience, preferably in oncology, palliative care, geriatrics, or home health care
  • Experience working with interdisciplinary teams preferred
  • Current Cardiopulmonary Resuscitation (CPR) certification (American Heart Association)
  • Current driver's license and insurance
  • Current, unrestricted LPN license in the state of Florida
  • Expertise in various technologies, including Microsoft Office and electronic medical records
  • Demonstrated ability to work non-judgmentally with persons of varied cultural, religious, and socioeconomic backgrounds and lifestyles
