Overview
Care Coordination is a vital role within the Mental Health Association of Columbia-Greene Counties, Inc., dedicated to advocating for the health and well-being of individuals, families, and communities. As a Care Coordinator, you will be part of a dynamic team that provides coordination of behavioral and/or physical healthcare for Medicaid-eligible individuals.
Job Description
The primary responsibilities of this position include coordinating with service providers and health plans to secure necessary care for members, conducting assessments and ongoing care management duties, providing transportation to clients when necessary, and linking/refer clients to needed services to support care plan/treatment goals. You will also conduct case reviews with an interdisciplinary team to monitor/evaluate client status/needs, advocate for services, and assist with scheduling of needed services.
Salary & Benefits
This full-time position offers a competitive salary of $22.00 per hour, 40 hours per week, Monday through Friday 8:30 am - 4:30 pm. The main worksite is in Catskill NY, with an office located in Hudson, NY. Requirements
To be successful in this role, you must have a bachelor's degree in a social service or counseling discipline, such as social work, psychology, nursing, rehabilitation, community mental health social work, psychology, or related field, or be a Registered Nurse with a bachelor's degree and at least 2 years' experience in providing direct services in the mental health field. Experience in providing direct service in the mental health or social service field and linking clients to services in the community is necessary. Proficiency in online technology systems is critical, and you must be able to work with a diverse population and demonstrate a commitment to Diversity, Equity, and Inclusion.
