Job Summary

We are seeking a highly skilled and compassionate Clinical Social Worker to join our team at Tyler Holmes Memorial Hospital. As a key member of our interdisciplinary treatment team, you will play a vital role in providing exceptional care to our patients and their families.

Responsibilities
  • Conduct comprehensive psychosocial assessments and develop individualized treatment plans
  • Provide individual, group, and family therapy sessions, as well as teach psychotherapy sessions as needed
  • Collaborate with the treatment team to ensure seamless care coordination and discharge planning
  • Document patient progress and treatment plans in a timely and accurate manner
  • Make recommendations and address family and community resource issues in treatment team meetings
  • Participate in Interdisciplinary Treatment Team Meetings and contribute to the development of treatment plans
  • Develop and maintain open communication with patients, families, and treatment team members
Requirements
  • Licensed Master's in Social Work (LMSW), Licensed Clinical Social Worker (LCSW), or Licensed Professional Counselor (LPC)
  • Two to three years of clinical experience with older adults, with hospital or nursing home experience preferred
  • Strong communication and interpersonal skills, with the ability to maintain confidentiality and adhere to ethical guidelines
Physical Requirements
  • Ability to stand for 1 hour per day, sit for 2 hours per day, and walk for at least 1 hour per day
  • Good manual dexterity, eye-hand-foot coordination, and clarity of vision
  • Ability to lift up to 15 pounds, bend, squat, kneel, balance, and reach above head and shoulder
Salary and Benefits

$33.61 per hour, with a comprehensive benefits package including medical, dental, and vision insurance, as well as paid time off and retirement savings plan.
