Advocate Aurora Health is seeking a skilled and compassionate Registered Nurse (RN) to join our Home Health team in West Allis, Wisconsin.
Job Summary:We are looking for a dedicated and patient-focused RN to provide high-quality care to patients in their homes. As a Home Health RN, you will work collaboratively with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities:- Conduct thorough assessments of patients to identify their physical, emotional, and social needs.
- Develop and implement individualized care plans that address the patient's needs and goals.
- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Collaborate with the interdisciplinary team to coordinate care and services for patients.
- Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
- Document patient care and progress in a timely and accurate manner.
- Current RN licensure in the state of Wisconsin.
- Basic Life Support (BLS) certification.
- Valid driver's license and reliable transportation.
- 2+ years of experience in home health or a related field.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a team.
