Job Title: Home Care Registered Nurse

Trinity Health Of New England At Home is seeking a compassionate and skilled Home Care Registered Nurse to provide exceptional care to patients in their homes. As a Home Care RN, you will be responsible for delivering high-quality, patient-centered care, and collaborating with our interdisciplinary team to achieve optimal patient outcomes.

Job Summary:

We are looking for a dedicated and experienced Registered Nurse to join our team as a Home Care RN. In this role, you will provide primary nursing care to patients in their homes, working closely with our Population Health Case Managers and other healthcare professionals to develop and implement individualized care plans. Your expertise and compassion will make a meaningful difference in the lives of our patients and their families.

Responsibilities:
  • Conduct thorough assessments of patients' medical and functional needs, identifying areas for improvement and developing targeted care plans.
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination and communication.
  • Provide skilled nursing care, including wound care, medication management, and education on disease management and self-care.
  • Supervise and support Home Healthcare Aides and LPNs as needed, ensuring high-quality care delivery.
  • Document patient progress, identifying areas for improvement and making recommendations for care plan adjustments.
Requirements:
  • Graduate of an approved nursing education program, BSN preferred.
  • Licensure as a Registered Nurse in the State of Connecticut.
  • One (1) year experience as a professional acute care nurse.
  • Home care experience preferred but not required.
  • Current Driver's license and reliable transportation.
