About the Position

We are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a member of our team, you will conduct comprehensive health assessments in the comfort of patients' homes, using your expertise to identify and address health risks and develop personalized care plans.

Responsibilities
  • Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
  • Provide health education and counseling to patients and their families
  • Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
  • Document patient information and care plans accurately and efficiently
Requirements
  • Master's degree in nursing or a related field
  • Current RN and NP licensure in the state of practice
  • Board certification in a Matrix-approved specialty
  • Minimum 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records

Travel Requirements

As a member of our team, you will be required to travel to patients' homes and other locations to conduct assessments and provide care. A valid state driver's license and proof of adequate automobile insurance coverage are required.
