Job Title: Home Health Visiting Nurse

Responsibilities:
  • Provide intermittent skilled nursing services to patients in their homes and qualifying facilities
  • Assess and evaluate patient needs, identify mutually agreed-upon goals, and develop individualized patient care plans
  • Communicate patient status and needs to other disciplines, including agency Clinical Supervisors and referring physicians
  • Supervise and instruct non-professional home health aide staff in the provision of personal care to patients
  • Participate in discharge planning and ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provides Age-Friendly Care
Requirements:
  • Current unencumbered State professional Nurse License
  • Valid driver's license and good driving record
  • Ability to make sound professional clinical judgment and assess and document patient needs
  • Proficient clinical skills and excellent verbal and written communication skills
  • Proficiency in personal computer use, including email, clinical, word processing, spreadsheet, and presentation software
Preferred Qualifications:
  • One year of experience as a home care professional nurse and competence in performing home care comprehensive assessments
