Job SummaryWe are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As an LPN at Carmel Hills, you will play a vital role in providing exceptional care to our residents, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.
Responsibilities- Provide direct patient care, including administering medications, performing treatments, and monitoring vital signs.
- Conduct thorough assessments of residents' physical and emotional needs, developing and implementing individualized care plans to meet those needs.
- Collaborate with the healthcare team to coordinate care, communicate resident needs, and ensure seamless transitions.
- Document resident care, including progress notes, medication administration records, and other relevant information.
- Participate in quality improvement initiatives, contributing to the development and implementation of policies and procedures that enhance resident care and safety.
- Graduate of an approved LPN program in the practicing state.
- Current licensure as an LPN in the state of practice.
- Minimum of 2 years of experience in a Skilled Nursing Facility (SNF) or similar setting.
- Current BLS/CPR certification.
