About the Role

CareOne Home Health is seeking a highly skilled Physical Therapist Assistant to join our team in providing exceptional care to patients in their homes. As a Physical Therapist Assistant, you will work closely with our physical therapists to develop and implement personalized treatment plans that promote optimal patient outcomes.

Key Responsibilities
  • Provide direct physical therapy services to patients in their homes, following the guidance of our physical therapists and in accordance with the patient's plan of care.
  • Treat patients to relieve pain, develop or restore function, and maintain maximum performance, using a variety of techniques and modalities.
  • Direct and aid patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities, and prosthetic training.
  • Utilize physical agents, including heat, cold, water, light, electricity, ultrasound, and massage, as prescribed by the physical therapist.
  • Observe and report to the physical therapist and organization personnel the patient's reaction to treatment and any changes in the patient's condition.
  • Instruct patients in the care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
  • Maintain accurate and detailed records of patient treatment, including clinical notes and conference notes, which will be incorporated into the patient's clinical record in a timely manner.
  • Participate in interdisciplinary group and organization in-service education programs to stay up-to-date on best practices and industry developments.
  • Participate in quality assessment and performance improvement teams and activities to ensure the highest level of patient care.
Requirements
  • Graduate of an accredited physical therapy assistant program with an Associate's degree in science or applied science, approved by the American Physical Therapy Association.
  • Current certification or license as required by state law.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
