Job Summary

We are seeking a highly skilled Diabetes Educator to join our team at CommonSpirit Health Mountain Region. As a Diabetes Educator, you will play a critical role in providing high-quality patient care and education to individuals with diabetes.

Key Responsibilities
  • Teach diabetes education care skills to patients with Type 1 and Type 2 diabetes in class and individual sessions.
  • Maintain optimal program development, policies, marketing, and data outcomes.
  • Collaborate with a multidisciplinary team to provide quality care for patients enrolled in the diabetes program.
  • Act as a leader, innovator, and change agent in the pursuit of improved quality of patient care.
Qualifications
  • Registered Nurse - Registered Dietitian or Registered Nurse in the state of employment.
  • Associate Degree required.
  • Three years' experience in patient education.
  • One year experience in diabetic education.
  • Certified Diabetic Educator.
  • Medium Work - exert/lift up to 50 lbs. force occasionally, and/or up to 20 lbs. frequently, and/or up to 10 lbs constantly.
