Key Responsibilities
  • Develop and implement individualized care plans in collaboration with residents, families, and interdisciplinary care teams.
  • Supervise and mentor nursing staff, ensuring high-quality care and adherence to best practices.
  • Collaborate with physicians and other healthcare professionals to ensure seamless care coordination.
  • Conduct regular assessments and evaluations to ensure resident-centered outcomes.
  • Manage and maintain accurate records and reports, ensuring compliance with regulatory requirements.
Requirements
  • Current licensure as a Licensed Practical Nurse (LPN) or Registered Nurse (RN) in the practicing state.
  • Current CPR certification, with the ability to obtain certification within 90 days of hire.
  • Recent clinical experience, education, or specialty skills specific to geriatrics.
  • Excellent communication and leadership skills, with the ability to work effectively in a fast-paced environment.
