Job Summary

We are seeking a skilled LPN Per Visit to join our team at MedStar Health. As a Patient Care Specialist, you will provide direct and indirect patient care services under the supervision of a registered nurse. This is a rewarding opportunity to make a positive impact on the lives of our patients and their families.

Key Responsibilities
  • Assessment and Care Planning: Assess patient's physical, mental, and emotional status and changes in patient's support system. Develop and implement individualized care plans to meet the unique needs of each patient.
  • Direct Patient Care: Provide direct nursing care according to physician's plan of treatment, including administering selected treatments and medication, and performing procedures in accordance with agency standards of practice.
  • Education and Support: Provide emotional support and physical comfort and safety of patients. Assist with the rehabilitation of patients and encourage independence in activities of daily living according to patients' capabilities and potential.
  • Documentation and Communication: Evaluate patient and family response to the plan of care and effectiveness of teaching. Document patient/family progress toward goals and outcomes according to agency clinical record policy.
Requirements
  • Education: Graduation from an accredited program of Practical Nursing required.
  • Experience: 1-2 years Recent clinical experience in an acute care setting required. Home care experience preferred.
  • Licenses and Certifications: LPN - Licensed Practical Nurse - State Licensure Valid LPN license in the District of Columbia, Maryland or Virginia, or any combination as required based on work location(s) Upon Hire required. DL - Valid State Driver's License in good standing with a dependable vehicle for transportation Upon Hire required. CPR - Cardiac Pulmonary Resuscitation (includes BLS and NRP) for healthcare providers from either the American Heart Association (AHA) or American Red Cross within 90 Days required.
