About the Role:

We are seeking a highly motivated and compassionate Physical Therapist Assistant to join our team at Interim HealthCare of Montgomery County and Southern Maryland. As a Home Health Physical Therapist Assistant, you will work closely with patients in their homes, providing personalized physical therapy to help them regain their strength and mobility.

Responsibilities:
  • Provide home-based physical therapy to patients with mobility, strength, and functionality impairments
  • Work collaboratively with a multidisciplinary team, including RNs, LPNs, CNAs, HHAs, OTs, and SLPs, to develop and implement patient care plans
  • Conduct assessments, document progress, and modify care plans as needed
  • Assist patients with exercises and hands-on therapy to manage pain and increase mobility
  • Educate patients and their families on therapy goals, exercises, and self-care
Requirements:
  • Graduate of an accredited Physical Therapist Program and active PT license in MD
  • Minimum of 2 years of physical therapy experience, preferably in home healthcare
  • BLS Certification
  • COVID-19 Vaccination
  • Flu Vaccination
  • Knowledge of state and federal home health regulations
  • Strong clinical judgment, interpersonal skills, dexterity, and compassion
