MedAdventures, a division of Icon Medical Network, is seeking a highly skilled Registered Nurse (RN) to fill a full-time Long Term Care position in Illinois. As a key member of our team, you will play a vital role in providing exceptional patient care and reducing hospital readmissions.
**Key Responsibilities:**
- Conduct comprehensive assessments to identify patient needs and develop personalized care plans.
- Collaborate with the Transitional Care Team to ensure seamless care transitions and reduce hospitalizations.
- Coordinate with nursing staff, leadership, and the interdisciplinary team to develop and implement integrated care plans.
- Facilitate referrals and consultations to ensure patients receive necessary care and services.
- Monitor patient outcomes and evaluate the effectiveness of care plans.
- Provide education and training to employees, providers, and contracted personnel on the Model of Care.
**Requirements:**
- Current unrestricted RN license in the state of Illinois.
- Minimum of 3 years clinical care experience, with Skilled Nursing Facility/Long Term Care experience preferred.
- Case management experience is a plus.
- Strong nursing expertise in managing complex cases with minimal supervision.
- Knowledge of current standard medical practices and experience with Medicare/Medicaid.
- Valid license with vehicle and insurance.
**Benefits:**
- Target base salary: $85,000 annually.
- Monday-Friday, 8am-5pm working hours.
- No Weekends, No Holidays, No Call.
- Comprehensive benefits plan.
- Split time between two facilities (9 miles apart).
