The Care Coordination Associate is responsible for providing exceptional customer service and ensuring seamless patient care coordination. This role involves answering incoming calls and emails, confirming eligibility, and determining transportation requirements.
Key Responsibilities- Answer and handle incoming phone calls and other communication in a timely manner
- Copy, type, and distribute documentation as needed
- Interact with team members, healthcare providers, and patients to ensure effective communication
- Learn and develop proficiency with Access2Care's transportation software
- Assist with managing recurring, short-notice trips for eligible patients
- Adhere to all company policies and procedures
- High School Diploma or equivalent
- Experience in a high-stress, high-call-volume environment (preferably in healthcare)
- Prior dispatch or routing experience (a plus)
- Excellent communication, organizational, and problem-solving skills
- Proficiency in Microsoft applications (Word, Excel, and Outlook)
- Ability to multitask with minimal supervision
- Knowledge of medical terminology and services (a plus)
