Job Title: Travel LPN to West Coast SNFs

Job Summary:

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Travel LPN to West Coast SNFs. As an LPN, you will provide high-quality patient care to residents in skilled nursing facilities (SNFs) on the West Coast.

Responsibilities:

  • Deliver basic nursing care to residents in accordance with care plans as ordered by physicians.
  • Conduct ongoing assessments of residents to identify care needs and make needed revisions to the care plan based on changes in resident condition.
  • Effectively meet all care needs of residents through timely completion of assigned duties.
  • Responsible for a wide variety of nursing administrative duties.
  • Conduct rounds within the nursing department to ensure quality care of residents.
  • Accurately document nursing care notes in resident charts in a timely manner.
  • Perform medication administration duties in accordance with the care plan as ordered by physicians.
  • Monitor progress of residents and make suggestions regarding treatment plans which may further meet their care needs.
  • Observe resident behavior, vital signs, symptoms, responses to treatment, and report findings to nursing supervisors.
  • Document interventions and resident or family education efforts timely and accurately.
  • Complete resident incident reports upon identifying changes in the condition of residents.
  • Monitor residents' skin to identify the risk of pressure sores and promptly report to the nursing supervisors.
  • Place medication and refill orders with the resident pharmacies as needed.
  • Monitor the emotional and physical conditions of residents to ensure comfort and safety.
  • Perform shift change reports with the previous shift nurses and ensure proper follow-up.
  • Understand and follow proper procedures for admission, transfer, and discharge of residents.
  • Provide expectations and monitor performance of specific nursing staff.
  • Direct and develop the CNAs to ensure the delivery of high-quality care and services in accordance with all laws, regulations, and facility guidelines.
  • Delegate assignments and provide overall supervision to CNAs.
  • Effectively communicate guidelines and procedures to CNAs, Restorative Aides, residents, families, and physicians.
  • Provide overall supervision for timely responses to call lights; respond personally as needed.
  • Assist in providing on-the-job training for new CNAs and LPNs.
  • Assist in promoting a team environment through effectively communicating praise and recognition with the CNAs.
  • Monitor schedule adherence of staff for lunch and rest breaks, beginning and ending of shifts, and overtime.
  • Monitor job performance of CNAs and provide correction as needed.

Requirements:

  • LPN or LVN degree from an accredited Nursing School.
  • Current licensure within the state of practice.
  • CPR certification required.
  • Minimum of one year nursing experience in a healthcare setting.
  • Geriatrics experience preferred.
