About the Role

We are looking for a motivated and compassionate Nurse Practitioner to perform consultations in various environments, including nursing homes and assisted living facilities. As a member of our team, you will work closely with our team of nurse practitioners and physicians to provide comprehensive care to our patients.

Responsibilities

• Provide high-level medical and mental health care to patients in Assisted Living, Independent Living, Skilled Nursing Facilities, and home settings.
• Work closely with our team of nurse practitioners and physicians to provide comprehensive care to our patients.
• Develop and implement treatment plans to meet the unique needs of our patients.
• Provide education and support to patients and their families on best practices for health and wellness.
• Master of Science in Nursing with FNP, ANP, AGNP, AGACNP, AGPCNP, or ACNP certification.
• Active License in South Carolina.
• DEA registration.
• Excellent communication and documentation abilities.
• Strong patient assessment skills.
