Key Responsibilities:- Develop and revise patient care plans in collaboration with the care team.
- Coordinate patient care activities and manage clinical data throughout the patient care continuum.
- Provide emotional support, coping support, and care for significant life changes.
- Communicate effectively with patients, families, and healthcare teams.
- Collaborate with clinical staff to provide care and referrals.
- Lead process and quality improvement activities to provide the best possible care.
- Prepare timely and accurate documentation and reports.
- Deliver care with sensitivity and respect, advancing care through development of new ideas and technology.
- Help coordinate and facilitate patient participation in research and clinical trials.
- Consult with physicians and other healthcare providers regarding patient care issues and quality of care.
- Participate in quality improvement measures and projects as requested.
- Bachelor's Degree Required
- Master's Degree Preferred
Licenses and Certifications:
- Current, unrestricted California RN license required
- BLS from the American Heart Association or American Red Cross required
- Ambulatory Nurse Certification or other specialty nursing certification required (or willing to obtain within one year from start date)
Experience:
- Must have 3 years experience in a pediatric setting (congenital heart, cardiology preferred)
- Preferred experience with program development, leadership role, implementation of navigator programs
- Required: Able to provide appropriate family-centered patient care
- Strong customer satisfaction, analytical, and problem-solving skills
- Able to use computer and software necessary for the position (e.g., Word, Excel, Power Point, Access); operate/troubleshoot basic office equipment required for the position, EMAR: EPIC
Physical Demands:
- Able to push, pull, lift or carry 0-35 pounds intermittently
- Able to sit, stand, walk, bend, and squat intermittently
