Key Responsibilities:

  • Assist residents with personal care and hygiene, including bathing, showering, and feeding, in accordance with individual care plans. Manage drainage collection devices and report output to the Nursing Team Leader.
  • Provide support for residents during bathroom visits, utilizing appropriate transfer techniques as outlined in care plans.
  • Administer oral and denture care as per facility protocols and directives.
  • Ensure residents are repositioned in bed and seated properly in chairs according to facility guidelines, including elevating heels as needed.
  • Respond promptly to call lights and assist residents as required, notifying the Nursing Team Leader of any requests needing a licensed nurse's attention.
  • Complete Stop and Watch forms and communicate any changes in residents' conditions to the Nursing Team Leader.
  • Demonstrate proficiency in obtaining and documenting vital signs, weight, and intake/output as directed, reporting findings to the Nursing Team Leader in a timely manner.
  • Assist with maintaining cleanliness in residents' rooms, including organizing personal hygiene items and ensuring a tidy environment.
  • Clean personal mobility equipment (e.g., wheelchairs, walkers) as necessary.
  • Document care provided in the tracking system accurately.
  • Support residents in attending activities and may assist in the execution of activity programs as needed.
  • Prepare residents' rooms for new admissions, orienting them to their surroundings and providing necessary supplies.
  • Assist residents with dressing and packing during discharge, ensuring rooms are cleared of supplies, trash, and linens.
  • Organize tasks effectively to ensure timely completion of responsibilities.
  • Collaborate with team members in the interdisciplinary care planning process.
  • Uphold resident confidentiality and adhere to HIPAA regulations.
  • Exhibit professionalism in interactions with peers, staff, and residents.
  • Perform additional duties as assigned.
