Key Responsibilities

  • Manage complex caseloads and prioritize tasks to ensure timely and effective service delivery
  • Develop and maintain relationships with community partners to ensure seamless referrals and services
Requirements
  • Master's Degree in Social Work (MSW) from a Council on Social Work Education (CSWE) accredited program
  • Minimum of two years of professional social work experience in a healthcare or community health setting
  • Licensed Clinical Social Worker (LCSW) or eligible for licensure (LCSW-A)
  • Strong written and verbal communication skills, with ability to network and access resources
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities
