### Job Description:

**Rehabilitation Expert**

We are seeking an experienced Physical Therapist (PT) to join our team at Aequor Technologies. This is a rewarding opportunity to provide in-home physical therapy services to patients recovering from illness, surgery, or injury.

### Job Responsibilities:

* Provide personalized physical therapy services to patients in the comfort of their own home, helping them regain mobility and independence.
* Conduct comprehensive assessments to develop treatment plans that cater to each patient's unique needs.
* Design and implement therapeutic interventions, including exercises, gait training, and mobility techniques, to improve patient strength, coordination, and range of motion.
* Educate patients and their families on home exercise programs, techniques for improving mobility, and safety measures to prevent falls or injuries.
* Collaborate with physicians, nurses, and other healthcare professionals to ensure a coordinated and comprehensive approach to patient care.
* Regularly assess and monitor patient progress, adjusting treatment plans as needed to ensure optimal recovery and achievement of therapy goals.
* Document patient evaluations, treatment plans, and progress notes in the electronic health record (EHR), ensuring compliance with home health regulations and standards.

### Requirements:

* Strong understanding of physical therapy principles, rehabilitation techniques, and home health care protocols.
* Excellent communication and interpersonal skills for working with patients, families, and healthcare teams in a home health environment.
* Proficiency in conducting assessments, creating individualized therapy plans, and implementing physical therapy interventions.
* Active [STATE] Physical Therapist (PT) License in good standing.
* A minimum of 2 years of experience as a Physical Therapist (PT), preferably in a home health or rehabilitation setting.
* Completion of an accredited physical therapy program (Master's or Doctoral degree).
* Certification in Basic Life Support (BLS).
* Experience with home health documentation systems (e.g., EPIC, Homecare Homebase, etc.).

### Compensation:

The estimated gross pay for this position is approximately $22,048.80 per week, based on the location and duration of the contract.
