Job Description
We are seeking an experienced Physical Therapist to join our team as a Home Health Rehabilitation Specialist. As a key member of our multidisciplinary healthcare team, you will be responsible for providing personalized physical therapy services to patients in their homes. Your primary focus will be on performing diagnostic tests and measurements, implementing individualized care plans, and educating patients and their families on rehabilitative care and activities necessary for independent living.

Required Skills and Qualifications
To be successful in this role, you must have a current MD Physical Therapist license, a minimum of one year of recent work experience as a Physical Therapist, and graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association. You should also possess strong interpersonal, organizational, and problem-solving skills, as well as solid computer skills with prior experience with electronic medical records (EMR) preferred.
