Job Title: LPN - Roswell GA

Job Summary:

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at St. George Village. As an LPN, you will play a vital role in providing high-quality care to our residents, working closely with our healthcare team to ensure the best possible outcomes.

Responsibilities:

  • Manage and supervise Certified Nursing Assistants (CNAs) regarding all areas of resident care.
  • Assist with health-related resident concerns as needed.
  • Administer medications as ordered by physician and document according to policy.
  • Monitor residents for changes in condition and notify appropriate personnel.
  • Collaborate effectively with other healthcare providers.
  • Implement resident care plans and modify changes plans of care according to changes in residents' condition.
  • Complete new resident nursing assessments for move-ins to the neighborhood and initiate the nursing & CNA care plans.
  • Communicate relevant information from new resident assessments to care partners.
  • Supervise shift rounds by CNAs.
  • Monitor &/or perform assigned tasks for shift.
  • Oversight of resident assignments of the CNAs.
  • Facilitate pharmacy communication for medication refills, medication changes, and stat orders.
  • Conduct audits of the medication administration record (MAR) as assigned.

Requirements:

  • Current Georgia LPN License with at least two years of related experience.
  • Possess good communication and inter-personal skills, documentation, and assessment.
  • The ability to read, write and follow written and oral instructions in English.
  • Current CPR training required.

Work Environment:

The work week can be up to 40 plus hours per week on a year-round basis. However, the position requires a willingness and flexibility in work hours when necessary to fulfill job responsibilities, including occasional evening or weekend work.
