Public Health Social Worker I/II: Home Visitation Program

Job Summary:

We are seeking a skilled Public Health Social Worker I/II to join our team in the Home Visitation Program. The successful candidate will provide comprehensive case management services to eligible applicants under the Maternal, Infant and Early Childhood Home Visitation Program.

Key Responsibilities:

  1. Evaluate clients' social, physical, and mental functioning through interviews, home visits, and record reviews;
  2. Determine clients' needs and provide case management services, including referrals to other programs or agencies as necessary;
  3. Screen applicants and refer those with complex social factors to other programs or agencies;
  4. Interpret departmental policies, rules, and regulations for service applicants, clients, and the public;
  5. Prepare and maintain case reports, records, and answer correspondence;
  6. Conduct group presentations and workshops as required;
  7. Perform other assigned duties as required.

Requirements:

  • Bachelor's degree with 30 semester (45 quarter) units in social work, counseling, psychology, or a closely related field;
  • One year of full-time experience as a social worker, preferably in a public health setting;
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations;
  • Knowledge of community resources and social services programs available at the state and community levels;
  • Ability to obtain and recognize relevant and significant facts, and to establish rapport with clients and their families.

Preferred Qualifications:

  • Knowledge of CalWORKs program and child development principles;
  • Experience working with vulnerable populations, including pregnant and parenting teens, and families with complex social factors.
