Role: Hospice LPN, serving various communities.
Key Responsibilities:
- Implement the care plan established by the lead RN.
- Educate patients and caregivers to empower informed health decisions and recognize potential health issues.
- Encourage independence in patients and families regarding their care responsibilities.
- Deliver comprehensive physical, emotional, and spiritual support.
- Assess home and family safety, providing suggestions for hazard mitigation.
- Apply the nursing process to execute care plans for patients and their families.
- Administer emergency interventions as necessary.
- Participate in community health assessments and health education initiatives.
- Maintain personal well-being, including physical stamina and mental health.
Qualifications:
- Graduation from an accredited NLN Practical Nursing program and completion of a pharmacology course.
- 2 to 3 years of recent experience in acute care nursing or similar roles; community health nursing experience is a plus.
- Valid LPN license in the State of Maine and current CPR certification.
- Possession of reliable transportation, insurance, and a valid Maine Driver's license.
