About the Role

We are seeking a skilled Registered Nurse to join our Hospice division in West Michigan. As a key member of our team, you will provide high-quality patient care and support to individuals in need.

Responsibilities
  • Conduct ongoing assessments of patients' physical and psycho-social status
  • Develop and implement individualized plans of care in collaboration with the Interdisciplinary Group (IDG), patients, and attending physicians
  • Provide education and interventions to support effective pain management and symptom control
  • Ensure timely and accurate documentation of patient services
  • Participate in IDG meetings and contribute to the development of patient care plans
Requirements
  • Active, unrestricted RN license in the State of Michigan
  • At least one year of relevant experience as an RN
  • Preferred qualifications include management experience, home health experience, and community health experience
  • Current CPR certification and reliable transportation with a valid state driver's license and current car insurance
