VITAS Healthcare is seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Nurse. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals and their families facing terminal illness and bereavement.
Key Responsibilities- Coordinate care and ensure continuous assessment of patients' and families' needs
- Develop and implement individualized plans of care in collaboration with the interdisciplinary team
- Provide education and training to patients, families, and caregivers on end-of-life care and symptom management
- Participate in patient care meetings and contribute to the development of care plans
- Collaborate with other healthcare professionals to ensure seamless care transitions and coordination
- Document patient information accurately and thoroughly in accordance with VITAS policies and procedures
- Current licensure to practice nursing in the state where the VITAS program is located
- Minimum of two years of nursing experience in hospice, home health, or community health
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Strong problem-solving and critical thinking skills
