About the Role

We are seeking a highly motivated and compassionate Case Aide to join our Care Coordination team at the Mental Health Association in Orange County, Inc. (MHA). As a Case Aide, you will play a vital role in providing support and assistance to our Care Coordination Staff and Program Participants enrolled in Care Coordination Services.

Key Responsibilities
  • Attend bi-weekly Care Coordination team meetings to ensure seamless communication and collaboration.
  • Participate in trainings and meetings relevant to Health Homes and Health Home Plus to stay up-to-date on best practices.
  • Assist with the delivery of care coordination services as outlined under the Health Home Model for Care Coordination.
  • Ensure that Care Management Staff maintain accurate and up-to-date client records electronically through Foothold Technology and FCM Electronic Record Keeping.
  • Provide transportation and logistical support to Program Participants as needed.
  • Assist Program Participants in accessing community resources that address social determinants of health, such as food pantries, employment opportunities, and physical activities.
  • Collaborate with the Intake and Outreach Specialist to complete Intake Assessments.
  • Establish and maintain relationships with community agencies and service providers to ensure a comprehensive support network for our Program Participants.
  • Represent MHA at meetings as necessary to promote our mission and values.
Requirements
  • A High School diploma (Associate's degree preferred)
  • A Credentialed Alcoholism and Substance Abuse Counselor (CASAC) certification
  • A valid driver's license and access to a reliable, personally owned vehicle
  • Two years of experience providing direct services to individuals with mental disabilities, developmental disabilities, alcoholism, or substance abuse
  • Experience linking individuals to a broad range of services essential to successfully living in a community setting
  • Bilingual Spanish/English preferred
  • Effective written and oral communication skills
  • Experience with Health Homes and Electronic Health Record Programs highly preferred
