Job Title: Home Health Case Manager RN - Day Shift

At HexaQuEST Health, Inc., we are seeking a skilled and compassionate Registered Nurse to join our team as a Home Health Case Manager RN - Day Shift. As a key member of our interdisciplinary team, you will work closely with physicians and other healthcare professionals to provide high-quality, patient-centered care to individuals in their homes.

Job Summary:

We are looking for a dedicated and detail-oriented Registered Nurse with a passion for home health care to join our team. As a Home Health Case Manager RN - Day Shift, you will be responsible for assessing patient needs, developing and implementing individualized care plans, and coordinating with healthcare providers to ensure seamless care transitions.

Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' unique needs
  • Coordinate with healthcare providers to ensure timely and effective care transitions
  • Provide skilled nursing care and support to patients and their families
  • Collaborate with interdisciplinary team members to ensure comprehensive care
Requirements:
  • Current RN licensure in the State of California
  • One year of experience as a professional nurse in a medical-surgical facility within the last three years or one to two years of recent home health care experience
  • Current CPR certification
  • Ability to work independently and as part of a team
  • Strong communication and interpersonal skills
