Key Responsibilities:

  • Conduct comprehensive assessments to identify patient needs and develop individualized care plans.
  • Coordinate and implement care plans, ensuring seamless transitions and continuity of care.
  • Collaborate with healthcare teams to ensure patient safety, satisfaction, and length of stay management.
  • Provide education and support to patients and families, promoting self-advocacy and empowerment.
  • Stay up-to-date with departmental and organizational goals, objectives, standards of performance, and policies and procedures.

Requirements:

  • Graduate of an accredited school of nursing, BSN preferred OR Masters Degree in Social Work (MSW) required.
  • Licensed as a Registered Nurse to practice nursing in the state of New Mexico.
  • Significant knowledge and understanding of availability of community and post-acute resources.
  • Two years' experience in a healthcare setting, with at least one year in an acute care setting.

Work Environment:

This role requires a general office environment and general hospital department/nursing units (100% of the day). Ability to move around the hospital to all units/department for about 80% of the day, in office 20% of the day. Requires close work, good vision, dexterity to write as well as to use a computer keyboard.
