About the Role

We are seeking a skilled Registered Nurse to provide oversight of care at our memory care and assisted living communities in Oshkosh and Wausau, WI. As a key member of our leadership team, you will be responsible for ensuring our residents receive the highest level of care at all times.

Key Responsibilities
  • Assess residents prior to admission and during their stay to ensure they receive appropriate levels of care.
  • Utilize the nursing process to develop and implement individualized care plans.
  • Oversee medication management, including ordering and administering medications as needed.
  • Communicate with resident POAs and primary care physicians to ensure accurate and timely medication changes.
  • Monitor electronic charting, including observations, incident, and fall reports.
  • Participate in auditing systems and regulatory processes as directed.
Requirements
  • Bachelor's or associate degree in nursing preferred.
  • Current, unencumbered active license to practice as an RN or LPN in the state of Wisconsin or qualifications to obtain RN or LPN license in Wisconsin prior to first day of employment.
  • One year of long-term care experience preferred.
