Bridge Home Health and Hospice is seeking a dedicated and compassionate Physical Therapist to join our team. As a Physical Therapist, you will play a vital role in providing high-quality patient care and contributing to our organization's mission of delivering exceptional healthcare services.
Responsibilities- Assess and evaluate patients' therapeutic, rehabilitative, and functional status, and participate in the development of the total plan of care.
- Provide physical therapy services to patients according to a written physician's plan of care and as defined in the state Physical Therapy Practice Act.
- Direct physical therapy treatment, instruct patients and families/caregivers in the use and care of therapeutic appliances, and determine priority needs for physical therapy.
- Report to physicians patients' reaction to treatment or changes in condition, and initiate physical therapy programs.
- Train patients in the use of prosthetic devices, identify patient and family/caregiver needs for other home health services, and refer as necessary.
- Prepare and submit clinical and progress summaries based on the attainment of goals, participate in discharge planning for patients, and provide physical therapy consultation to home families/caregivers when indicated.
- Supervise physical therapy assistants according to organization policy and state regulations, and instruct, supervise, and evaluate home health aide care every two (2) weeks.
- Possess a post-baccalaureate degree in physical therapy (Doctor of Physical Therapy) approved by the Commission on Accreditation in Physical Therapy Education (CAPTE).
- Be licensed to practice as a physical therapist by the Physical Therapy Board of California.
- Have two years of appropriate experience as a physical Community/home health experience is preferred.
- Demonstrate good verbal and written communication and organization skills, possess and maintain current CPR Certification, and possess and maintain valid and current Auto Insurance.
