**Job Summary**

KSDT CPA is seeking a skilled and compassionate Hospice Social Worker to provide emotional support, counseling, and community resource coordination for patients and their families facing terminal illness.

As a vital member of the interdisciplinary team (IDT), the Hospice Social Worker helps ensure patients receive holistic, person-centered care that honors dignity, preferences, and quality of life.

**Key Responsibilities**

  • Conduct comprehensive psychosocial assessments within 5 days of admission and as needed.
  • Participate actively in the interdisciplinary group (IDG) to help develop and revise individualized plans of care.
  • Provide emotional and psychological support to patients and families throughout the end-of-life journey.
  • Assist families with advance care planning, funeral planning, and navigating difficult decisions.
  • Coordinate access to community resources such as financial assistance, housing, food, transportation, and caregiver support.
  • Provide crisis intervention and grief support before and after death, including participation in the bereavement program.
  • Educate patients and families about hospice philosophy, services, and their rights.
  • Maintain timely and accurate documentation in the electronic health record (EHR).
  • Collaborate with chaplains, nurses, physicians, and other team members to ensure quality, coordinated care.

**Qualifications**

  • Bachelor's Degree in Social Work (BSW) required; Master's Degree in Social Work (MSW) preferred.
  • Licensed Social Worker (LSW) or Licensed Clinical Social Worker (LCSW) in the State of [insert state].
  • Minimum of one (1) year of experience in a healthcare or hospice setting preferred.
  • Strong communication, problem-solving, and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Valid driver's license and reliable transportation for home visits.
