Bristol Hospice is seeking a highly skilled and compassionate Advanced Practice Registered Nurse (APRN) to join our team as a Nurse Practitioner in Hospice Care. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals and families in need of end-of-life support.
Key Responsibilities- Provide Comprehensive Hospice Care: Evaluate and assess patient health history, health promotion, and medical conditions; manage health problems by directing and developing the plan of care, prescribing medications or treatments, and coordinating with patients, families, and other healthcare practitioners.
- Collaborate with Interdisciplinary Team: Work closely with the hospice medical director, attending physicians, and other healthcare practitioners to ensure seamless care coordination and patient-centered decision-making.
- Lead Clinical Initiatives: Develop, implement, and evaluate clinical protocols and guidelines to improve patient outcomes and quality of care.
- Communicate Effectively: Educate patients, families, and caregivers on preventative care, medical problems, psychological problems, and spiritual problems in conjunction with the interdisciplinary team.
- Ensure Compliance: Maintain knowledge of and compliance with current Medicare/Medicaid, state/federal rules and regulations for hospice services, as well as Bristol Hospice's policies and procedures.
- Education and Experience: Master's degree in Nursing with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
- Certification and Licensure: Current nursing licensure in the state and accreditation from an approved certifying body for advanced practice nursing as required by the state.
- Skills and Abilities: Strong clinical skills, excellent communication and interpersonal skills, ability to work in a fast-paced environment, and commitment to providing high-quality patient care.
