As a team member, you'll be part of a dynamic group working together to provide care to patients in their homes, nursing homes, and assisted living facilities.
Key Responsibilities:- Primary Care Delivery: Provide cost-effective, quality care to assigned members, managing both medical and behavioral conditions effectively.
- Care Coordination: Understand Payer/Plan benefits, CCM associate policies, and articulate them effectively to providers, members, and key decision-makers.
- Program Enhancement: Regularly communicate with internal and external parties, promote the CCM program, and develop new methods to address concerns.
- Certified Nurse Practitioner through a national board.
- Graduate of an accredited master's degree in Nursing (MSN) program and board certified through the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
- Active and unrestricted license in the state of residence.
- Current active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations.
- 1+ years of hands-on post-grad experience within Long Term Care.
- Understanding of Geriatrics and Chronic Illness.
- Understanding of Advanced Illness and end-of-life discussions.
