As a Licensed Practical Nurse (LPN) with Fort Atkinson Health Care Center, you will play a crucial role in ensuring that our patients receive the necessary care and that we maintain the resources required to provide it. Responsibilities:
- Supervise Certified Nursing Aides under the direction of an RN.
- Collect patient care data and monitor patient conditions and family dynamics based on observation, history, and nursing/medical diagnosis.
- Contribute to the development of the care plan and discharge plan for assigned patients.
- Implement patient care utilizing the care plan, delegated medical orders, and collaborate with health team members as necessary.
- Assist in evaluating outcomes of patient care planning and teaching, revising the plan according to the changing needs of the patient.
- Document the delivery of healthcare and nursing processes in accordance with specific unit standards and hospital nursing policy.
- Administer and document medications, internal nutrition, and treatments per physician's orders, accurately recording all care provided.
Qualifications:
- Minimum requirements include graduation from an accredited school of licensed vocational nursing or licensed practical nursing with a current state license in good standing.
- Two or more years of acute care or rehabilitation experience as an LPN is preferred.
- Current state license to practice as an LPN, with maintenance of current provider CPR certification throughout employment. ACLS certification is preferred.
