As a Registered Nurse on our Home Hospital team, you will be responsible for delivering patient-centered care, collaborating with interdisciplinary teams, and promoting patient safety and well-being.
Key Responsibilities:- Develop and implement individualized care plans that meet the unique needs of each patient.
- Provide comprehensive nursing care, including assessment, education, and treatment, in a home-based setting.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Monitor patient progress, identify potential issues, and make adjustments to care plans as needed.
- Document patient information accurately and timely, using electronic health records and other tools.
- Participate in ongoing education and training to stay up-to-date on best practices and industry developments.
- Current Massachusetts license as a Registered Nurse.
- Minimum of one year of recent nursing experience in an acute or emergency setting.
- Home care experience preferred.
- Ability to work independently and as part of a team.
- Strong communication and interpersonal skills.
- Ability to work with diverse patient populations and adapt to changing situations.
