About the Role

We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team at Optum, a leading healthcare organization dedicated to delivering high-quality, patient-centered care. As a member of our interdisciplinary team, you will play a critical role in providing comprehensive medical care to our patients in the comfort of their own homes.

Key Responsibilities
  • Conduct initial comprehensive assessments and provide ongoing care to patients
  • Develop and implement patient management care plans, updating them as needed
  • Collaborate with patients, their legal representatives, and other healthcare professionals to ensure seamless care delivery
  • Prescribe diagnostics and interventions to prevent unnecessary hospital admissions
  • Complete follow-up and post-discharge assessments, consulting with hospital and emergency teams as needed
  • Educate patients and their representatives on disease management and wellness strategies
  • Participate in ongoing meetings to promote clinical excellence and quality care
  • Implement HEDIS measure campaigns and quality initiatives to drive exceptional patient outcomes
Requirements
  • Completion of a Nurse Practitioner or Physician Assistant accredited program
  • Current state RN/NP or PA license
  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
  • Access to reliable transportation
Preferred Qualifications
  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
  • Knowledge of long-term care, community resources, and cost-effective alternatives
  • Knowledge of CPT, ICD-10, and HCPC codes
  • Knowledge of clinical standards of care
  • Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
