We are seeking a highly skilled Nurse Practitioner or Physician Assistant to join our team at Optum, a leading healthcare organization dedicated to delivering high-quality, patient-centered care. As a member of our interdisciplinary team, you will play a critical role in providing comprehensive medical care to our patients in the comfort of their own homes.
Key Responsibilities- Conduct initial comprehensive assessments and provide ongoing care to patients
- Develop and implement patient management care plans, updating them as needed
- Collaborate with patients, their legal representatives, and other healthcare professionals to ensure seamless care delivery
- Prescribe diagnostics and interventions to prevent unnecessary hospital admissions
- Complete follow-up and post-discharge assessments, consulting with hospital and emergency teams as needed
- Educate patients and their representatives on disease management and wellness strategies
- Participate in ongoing meetings to promote clinical excellence and quality care
- Implement HEDIS measure campaigns and quality initiatives to drive exceptional patient outcomes
- Completion of a Nurse Practitioner or Physician Assistant accredited program
- Current state RN/NP or PA license
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or National Commission on Certification of Physician Assistants (NCCPA)
- Access to reliable transportation
- 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care
- Knowledge of long-term care, community resources, and cost-effective alternatives
- Knowledge of CPT, ICD-10, and HCPC codes
- Knowledge of clinical standards of care
- Awareness of NCQA requirements, CMS guidelines, Medicaid/Medicare contracts, and benefit systems
