We are seeking a highly skilled and compassionate Registered Nurse to join our team as a Full-time Registered Nurse Case Manager for our Home Health division in the greater Philadelphia area.
Key Responsibilities- Provide primary nursing care to home-based care patients as ordered by the physician, and in accordance with our organization's policies and procedures.
- Develop and implement individualized care plans in collaboration with our Population Health Case Manager.
- Provide functional support and supervision to Home Healthcare Aides and LPNs as needed.
- Deliver therapeutic interventions, overall care management, and achieve top decile client outcomes and patient satisfaction.
- Graduate of an approved nursing education program.
- Licensure as a Registered Nurse in the state of PA.
- One (1) year experience as a professional care nurse.
- Current Driver's license and reliable transportation.
