We are seeking an experienced Advanced Clinical Pharmacist Critical Care to join our team at Methodist Hospital Metropolitan. As a critical care pharmacist, you will play a vital role in providing high-quality patient care and contributing to the success of our hospital.
Key Responsibilities- Interpret orders and accurately transcribe or verify them into the pharmacy computer system
- Accurately compound and dispense pharmaceuticals
- Comply with facility policy and procedures and regulatory requirements
- Monitor drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose
- Provide clinical consultation and clarification to practitioners and suggest appropriate, cost-effective therapeutic alternatives
- Sustain the hospital formulary, minimizing nonformulary procurements, and promote rational drug therapy selection
- Attend and actively participate in daily multidisciplinary and bedside rounds
- Support the hospital formulary decision process by working with medical staff to promote rational drug therapy and make improvements identified by drug utilization evaluations
- Provide accurate and timely medication information to healthcare staff and patients
- Actively participate in pharmacy clinical services such as drug consults, nutrition support, pharmacokinetics, anticoagulant monitoring, pain management, IV to PO, renal dosing, and other programs as defined by Pharmacy Leadership
- Doctor of Pharmacy (PharmD) or Bachelor of Science in Pharmacy
- One of the following is required: Minimum of 1 year of hospital clinical pharmacy experience or at least 2 years of general hospital pharmacy experience required; preferably within area of focus
- Completion of hospital PGY1 / pharmacy general practice residency or fellowship
- BCPS certification or other Board of Pharmacy Specialty (BPS) certification
- Current Registered Pharmacist licensed by the Texas State Board of Pharmacy or Licensed pharmacist in another state: Provide proof of the Authorization to Test (ATT) from the Texas State Board of Pharmacy
