Job Summary

We are seeking a highly skilled Speech-Language Pathologist to join our team at Clarksville Health & Rehab Center. As a Speech-Language Pathologist, you will play a vital role in helping our patients achieve optimal physical function and facilitating their achievement of patient, physician, and therapy goals.

Key Responsibilities
  • Screen and Evaluate Residents: Conduct thorough assessments to determine the need for Speech, Language, or Swallowing Therapy.
  • Develop and Implement Treatment Plans: Establish individualized treatment programs with consultation to facility staff in areas of Speech Therapy.
  • Communicate with Caregivers and Family Members: Provide results of evaluations and treatment objectives to caregivers, family members, and residents.
  • Document and Monitor Progress: Complete all documentation as required by the fiscal intermediary, within the designated time frame, and monitor the resident's response to treatment, altering the treatment plan as appropriate.
Requirements
  • Master's Degree in Speech-Language Pathology: Employee must hold a Master's Degree in Speech-Language Pathology from an accredited college or university and be licensed accordingly.
  • Previous Experience in Long-Term Care: Previous experience in long-term care is preferred, but not required.
