About the Job

Providence Service is seeking a skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a vital role in providing high-quality, individualized, cost-effective pharmaceutical care and integrated pharmacy operational services in collaboration with the healthcare team to optimize patient's medication therapy outcomes and quality of life.

Responsibilities
  • Provide pharmaceutical care to patients, including medication therapy management and counseling.
  • Collaborate with the healthcare team to optimize patient care and outcomes.
  • Develop and implement pharmacy operational services to improve patient care and outcomes.
  • Participate in quality improvement initiatives to enhance patient care and outcomes.
  • Stay up-to-date with the latest developments in pharmacy practice and apply this knowledge to improve patient care and outcomes.
Requirements
  • PharmD - Pharmacy Doctorate or Bachelor's Degree - Pharmacy.
  • California Pharmacist Registered upon hire.
  • Where applicable, obtain a Board of Pharmacy Preceptor license.
  • Where applicable, Basic Life Support-Provider (BLS).
Preferred Qualifications
  • Acute care hospital pharmacy experience, 1 year or equivalent experience.
  • Completion of ASHP-accredited PGY1 residency.
