Purpose and Scope

The primary objective of this position is to provide and direct physical therapy services to patients, encompassing pediatric, adult, and geriatric populations. The goal is to facilitate patients' attainment of their maximum performance potential within a medical framework.

Education and Qualifications
  • Master's or Doctoral degree in Physical Therapy or equivalent work experience.
  • Successful completion of the State Certification Examination for Physical Therapy.
  • Ongoing commitment to continuing education and professional development.
  • Maintenance of valid state Physical Therapy licensure.
Performance Expectations

This role involves a range of specific duties, which are considered essential functions. These duties are subject to change as per the needs of our customers, patients, co-workers, and associated policies.

  • Provides physical therapy services to patients and initiates referrals as necessary, adhering to Physical Therapy Department Procedures.
  • Conducts regular screenings of residents to determine the need for interventions/treatment.
  • Evaluates residents to gather data necessary for treatment planning and implementation, including specialized evaluations as indicated.
  • Obtains appropriate physicians' orders/documentation for treatment.
  • Interprets and communicates evaluation findings to patients, families, and other team members.
  • Develops treatment plans by identifying target lists, including short and long-term goals and methods to achieve identified goals.
  • Coordinates, implements, and conducts treatment and plans with patients, family, and other team members.
  • Monitors patients' responses to intervention and modifies treatment as needed to attain goals in accordance with plans and physician orders.
  • Develops appropriate home or community programming to maintain and enhance patients' performance in their environment.
  • Will to work with severely physically, emotionally, and cognitively compromised patients.
HIPAA and the Minimum Necessary Standard

This role requires someone in good physical condition, with the ability to perform the following physical capabilities, with or without reasonable accommodations.

  • Use of proper body mechanics and techniques for bearing weight.
  • Ability to sit for extended periods.
  • Walk through the department, office, and other locations as necessary.
  • Lift objects weighing pounds or more.
  • Must be able to climb stairs, stoop, and reach when performing daily work.
  • Ability to assist and perform patient transfers safely and effectively with or without mechanical devices.
