Key Responsibilities
  • Assess and develop individualized care plans for patients in their homes
  • Provide direct patient care, including medication management and wound care

  • Practice within the scope defined by the Washington State Nurse Practice Act
Requirements
  • Associate's Degree in Nursing
  • Washington Registered Nurse License
  • National Provider BLS - American Heart Association
  • National Provider Identifier (NPI) number
Preferred Qualifications
  • Bachelor's Degree in Nursing
  • Three to Five (3-5) Years in Acute care or community behavioral health setting
  • Community behavioral health and geriatric experience
